Cataract Surgery
Cataracts represent a yellowing or opacification of the human lens. The lens, located behind the pupil, is one part of the eye which focuses light rays on the retina. As we age, the lens becomes progressively less clear and more difficult to see through. Over 90% of cataracts are related to normal aging. There are also certain congenital and acquired conditions of the eye as well as some medications which can lead to cataracts at any age. We define this loss of clarity of the lens as a “cataract” once it starts to become visually symptomatic. Patients tend to complain of decreased distance and /or reading vision, decreased night vision, loss of image clarity, dulling of colors and increasing glare around lights.

Dr.’s Elahi, Chen and Schwartz are proud to be using the most advanced techniques available for cataract surgery. This often includes topical anesthesia (no longer necessitating the need for a needle injection of anesthesia near the eye), stichless clear corneal incisions and phacoemulsification (ultrasonic waves used to break up the cataract). Once the cataract is removed a synthetic lens implant which is precisely measured for your eye is implanted.

Our doctors have trained extensively in the art of cataract extraction, an art which they teach to ophthalmology residents at the world class Mount Sinai Medical Center in New York City. Acrysof Toric Intraocular Lens
Astigmatism occurs when the cornea is shaped more like a cylinder (like the shape of a football) than a sphere (like the shape of a basketball). This shape leads to multiple images being cast on the retina and subsequent blurring of an image. While treatments such as LASIK or PRK are available for the treatment of astigmatism, prior to now there has been no lens implant that could be inserted after cataract surgery that would correct a person's astigmatism. Traditional lens implants do not correct for astigmatism and a person might continue to experience blurred or distorted vision after cataract surgery without the use of glasses or contact lenses.

For those patients with astigmatism who would like freedom from eyeglasses for distance vision after cataract surgery there is now a better option. The Acrysof Toric lens is a foldable lens that a surgeon implants after cataract surgery to replace the clouded lens (cataract). The unique design of the Toric lens makes it possible to significantly reduce or eliminate corneal astigmatism and significantly improve uncorrected distance vision.

Restor Multifocal Intraocular Lens
Dr.’s Elahi, Chen and Schwartz are amongst a handful of surgeons in Manhattan certified to implant the Restor multifocal intraocular lens after cataract surgery. At birth, the lens of the eye is not only clear but elastic and as a result the lens can change shape and power for near tasks such as reading. As we age this elasticity is lost, a condition called presbyopia, which usually begins in the fifth decade. Typically after the cataract is removed a lens implant is placed in the eye which can correct distance vision and glasses are needed for near tasks including reading, computer work, make-up application, knitting and various hobbies.

We now have advanced technology which allows implantation of a multifocal lens called the Restor which has the ability to restore not only distance but near vision. In the clinical study, 80% of patients who had a Restor implanted reported that they never wore glasses for distance or near activities. Corneal Transplant Surgery
The cornea is a transparent tissue, about one half millimeter thick, that is the window to the eye. It is responsible for two thirds of the light focusing power of the eye and its clarity is crucial to your vision. Many conditions can cause the cornea to become cloudy or misshapen, both of which can prevent good vision. Sometimes it can become so cloudy and vision can become so poor that a corneal transplant is necessary. In this procedure part of a person’s cornea can be removed and tissue from an organ donor can be surgically attached in the diseased tissue’s place. This procedure has returned vision in a way previously impossible for many patients. Much of the pioneering work done investigating this procedure was done as part of our practice at Fifth Avenue Eye Associates.

Glaucoma Surgery
Glaucoma represents a broad spectrum of diseases which share damage to the optic nerve as a common endpoint. It is a disease of the optic nerve which has many risk factors including but not limited to high eye pressure (though pressure can be normal), increasing age, race, family history, diabetes and corneal thickness.

The overall prevalence of open angle glaucoma in the United States, representing 90% of all glaucoma, is estimated to be 1.86%, with 1.57 million white and 398,000 black persons affected. Overall glaucoma affects 2.22 million US citizens. The number of individuals with open angle glaucoma in the United States will increase by 50% to 3.6 million in 2020, largely due to the aging population.

Primary Open Angle Glaucoma is the most common form of the disease in which the drainage of fluid from the eye is insufficient and the intraocular pressure is elevated. This pressure elevation over time can lead to damage to the optic nerve and possibly blindness.

Narrow angle glaucoma or acute angle closure glaucoma is a disease in which the drainage system of the eye is blocked by the iris (colored portion of the eye) thus preventing the egress of fluid and causing a dangerous rise in intraocular pressure.

Adults should be routinely screened for all types of Glaucoma as part of their annual eye exams and those with any of the above mentioned risk factors should be followed closely. Because the loss of vision caused by glaucoma is undetectable until a very advanced stage, screening exams are essential.

Peripheral laser iridotomy is performed in some cases of narrow angle glaucoma and acute angle closure. This is a painless in-office procedure in which a microscopic hole is made in the periphery of the iris to allow fluid to flow into the drainage system of the eye. Argon laser trabeculoplasty is performed in some cases of open angle glaucoma to facilitate drainage out of the eye and decrease intraocular pressure. In some cases a non-laser surgical procedure called a trabeculectomy is necessary in which a new drainage system is created surgically in the eye in order to decrease the intraocular pressure.

Advanced retinal and optic nerve imaging and analysis is performed at the fifth avenue eye associates including nonmydriatic fundus photography, Humphrey visual field analysis and Heidelberg Retinal Tomography. Our doctors have a wealth of experience treating both medical and surgical glaucoma and preventing its potentially devastating visual consequences.

Diabetic Eye Care
Diabetes has become a health epidemic in the United States. 20.8 million people or 7 percent of the population in the US have diabetes, 6.2 million of which are undiagnosed. Diabetes is a disease that hits the small blood vessels of the body. Most notable of these small blood vessels or end organ circulations are the kidney and the retina. Annual examination of the retina allows us to promptly diagnose small vessel disease of the retina. By finding these changes early, the doctors at fifth avenue eye associates can not only help prevent the vision threatening complications of diabetes, but also work with your general medical doctor to fine tune your diabetes regimen. Any changes made in diabetes care can be monitored via the human retina and the improvement or stability of a person’s diabetic retinopathy.

Advanced diabetic retinopathy may require laser eye treatment called Pan Retinal Photocoagulation or Focal Retinal Photocoagulation. Fifth Avenue Eye Associates is equipped with a state of the art Argon Laser used for this purpose and if necessary treatment can be done in the comfort of our facility.

Ocular Surface Reconstruction
Phototherapeutic keratectomy is a procedure similar to photorefractive keratectomy, done for laser vision correction, in which the excimer laser is used to reshape and revitalize a cornea that has certain types of corneal dystrophies or recurrent erosions of the ocular surface. Such conditions can be vision threatening as well as painful and debilitating. Modern laser treatment has allowed us to restore sight and quality of life to individuals in whom this was previously impossible.
